NOTICE:
All diver physical examinations and medical information are covered
by the Privacy Act and must be handled in accordance with NOAA
Directive 22-50 and good medical practice.
1.
The Unit Diving Supervisor (UDS) will provide the diver with all
forms required, including SF-88 & SF-93 or NF 56-58 &
NF 56-60, current NOAA Diving Program - Medical Examination Checklist
(NF 56-57), and Body Composition Screening Form.
2. Upon completion of their medical examination, individual divers
must submit a copy of the physical to the UDS for screening. The
UDS will forward complete physicals to the NOAA Diving Center
(NDC).

Upon receipt of the examination, the UDS shall review for completeness
the items on the physical exam listed on the NOAA Diving Program
- Medical Evaluation Checklist. The UDS shall request any additional
information necessary to complete the physical from the diver. Complete physicals shall be forwarded to NDC for review
and approval by the NOAA Diving Medical Officer (DMO) or designee.

NDC may approve or disapprove (on behalf of the Medical Review
Board) a diving physical based upon current NOAA Diving Medical
Standards. Reviewing Physicians may follow up on inconsistencies
or omissions in the examinations, or may return the exam to NDC
for follow up. Approved physicals will be annotated and entered
into the NOAA Diver Database at NDC. The monthly dive roster will
serve as notification. NDC will notify the appropriate UDS immediately
when a physical is incomplete or disapproved.

Physicals that require Medical Review Board action will be forwarded
by NDC to the Chairman of the NOAA Diving Medical Review Board
(NDMRB). NDC personnel will act under the direction of the Chairman
of the NDMRB towards resolving the case in a timely fashion. The
appropriate UDS and diver will be informed of the results as soon
as possible.
6.
Requests for waivers of the Medical Evaluation Criteria may only
be granted by the Director, NOAA Diving Program after consultation
with the Medical Review Board.
